2/9/2024 EnchantaVaughn Approved Behavior: Client started the session presenting herself in a safe and polite manner while displaying good conversation with her mom and sibling. Client greeted QMHP with a smile and a hug. Client was observed in decent clothes and well-kept hairstyle. Client’s hygiene was up to par. Client did not appear to be sad or upset over anything. Client’s room was organized and in order, which made it easy to have the session in her room instead in the living room. Client prepared to answer questions regarding her mental health. Mother reports she has to prompt her several times to complete chores within the home and when she cannot have her way, she gets upset with her (mother) and shuts down with behaviors lasting 10 minutes in duration. Behavior: Client was polite and courteous to mental health professional while reviewing her weekly goals. Client interacted well with her family and displayed excellent leadership skills by listening, paying attention, and sitting correctly on her bed. Client displayed appropriate behaviors while listening to QMHP go over treatment plan and expectations but at times she would get a little up set with QMHP and smack her teeth at which behaviors lasted 2-5 minutes in duration. Intervention: QMHP implemented and modeled new ways for client to maintain positive energy. QMHP supported client with coping with stressors without needing additional redirection or prompts. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the importance of her therapy. QMHP praised and recognized client for her hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent home routine. Intervention: QMHP explored positive behavior options she was exhibiting with client to support her willingness to continue to make good decisions and display appropriate behaviors in the home. QMHP processed thoughts and feelings with parent. QMHP encouraged client to continue expressing herself and letting go of unwanted tension between peers and authority. QMHP provided simple praises to client for her commitment to her progress and treatment. Response: Client was able to reevaluate herself and display a more positive attitude toward peers and authority. Client has become more involved in age appropriate social activities and was given positive reinforcement for these choices. Client was able to communicate with the QMHP as information was shared. Client communicated that she felt safe and comfortable talking with the QMHP about things that were personal and dear to her. Client shared that she believes she will be able to master her feelings and control her anxiety better. Response: Client was attentive and open with her counselor about things she felt needed to be addressed. Client demonstrated relaxation skills and responded well to feedback. Client interacted well within the home, indicating she was comfortable and in a good space. Client verbalizes positive feelings about herself and recognizes her improvements in the home. Client was happy today was the last day of virtual school and looked forward to the rest of summer. Client shared that she enjoyed today’s session and the time she spent with QMHP.
